Simply print off the labels and attach to four empty jars or containers and place your goals in each corresponding jar, they can be Goals you set at the beginning of the year and that arise throughout the year.

As you achieve a milestone take your goal and pop it in your achievement jar. At the end of the year open up your achievement jars and taker a look at all the amazing things you achieved, things both big and small, even the goals that are often taken for granted or seem small to others but are worth you celebrating.
